Aftercare Supportive Next Step Wraparound Transition Services


Aftercare wraparound transition services are based on a model of solution oriented service and support that develops plans focusing on the individual strengths and needs of members of the family. Plans center on the family, and are built upon the child's and family's unique strengths while bringing the services to the family in their community and in in their home.

The objectives are to provide varying levels of support until the family reaches a level of self sufficiency.

Active crisis and safety planning minimize uncertainty and fear which are normal feelings for all when a child is returning home from residential treatment. Through these services, families develop an effective support network, increase their sense of competence, acquire new skills for managing the ongoing special needs of their child, and have access to the supportive resources they need to build brighter futures for themselves. Commitment to the child and their family can be an incredibly supportive service - through all of life's little ups and downs. Staff are hired and trained for the sole purpose of supporting the needs of the child and their family.

Long-term goals are addressed with an objective Plan. Parents are encouraged to be actively involved in creating, carrying out and following through on the Plan.
