First Meeting

Welcome!

We look forward to working with your family.

A comprehensive academic, developmental, family intervention, medical and social history will need be completed and submitted before the first meeting. This will allow us to then evaluate and determine your most critical and important concerns and goals. The following list may assist us in this initial first meeting and aid us all to develop momentum. Please review the list below for suggestions of what to send ahead of time, when possible.

Please do not be too concerned when you do not have these things readily available.

You can collect these materials as we proceed.

We may also request information from important hospitals, medical personnel, programs, schools and professionals, as well as determine what information is even important or necessary. These issues can be discussed at the first meeting and plans to obtain these records can be made at that time.

Comprehensive History


Information, Records and Reports Which May Be Useful

Clinical

  • Drug and alcohol evaluations
  • Intervention records (previous hospitalizations, other therapies, programs, etc.)
  • Medication history - Current and past medications
  • Psychiatric and psychological evaluations
  • Risk assessments

Evaluations and Testing

  • ADD/ADHD records
  • Assessments (developmental, IQ, reading, sensory, verbal, etc.)
  • Educational or psychological evaluations (independent or school based)
  • Speech and Language evaluations and recommendations

School

  • 504 and IEP records
  • Learning evaluations
  • School records (incident reports, infractions, suspensions, etc.)
  • School transcripts as well as current grades
  • Teacher evaluations and/or recommendations (when needed)
  • Tutoring evaluations and summaries

Supplementary

  • Academic and/or Awards received
  • Art and/or poetry examples
  • Current health documents related to schooling or treatment
  • Current picture of child and/or child and family for our records
  • Professional contacts (attorney, physicians, probation officer, treatment providers, tutors, etc.)
  • Records of accomplishments or awards (Eagle Scouts, sports, etc.)
  • Writing sample

Academic / Psychological specialty testing may need to be considered and we will discuss this with you when necessary.
